According to Vogue, it is Kim Noorda - but I am not familiar with her so I can't be sure.

I remember this shot well, first time I saw it was in an issue of W from Sept 2003. They did a retrospective of 40 of Kate's most interesting shots.....pretty much solidified my appreciation of the skinny aesthetic!
