Most contracts have an appearance clause where the agency or client can terminate for any changes to the model's appearance. That isn't just weight, it's everything - tattoos, hair, piercings, etc. We're very clear with all of our models that you do not change anything without permission. Obviously if you're in demand and well-known you get more flexibility but I've never seen a contract where the model can do whatever she wants with no consequence.
